Section 34-25A-5 Application for licensure or registration. (a) Except as provided in subsection (h) or (i), no person shall administer prosthetic, orthotic, or pedorthic care in this state unless licensed or registered to do so in accordance with this chapter. The board shall issue approved forms for application prior to January 1, 2003. (b) In order to obtain a license as a prosthetist, orthotist, or prosthetist/orthotist in this state, an applicant shall be a citizen of the United States or, if not a citizen of the United States, a person who is legally present in the United States with appropriate documentation from the federal government, and shall do the following: (1) File a written application on forms to be developed and approved by the board. Section 8-17-100 (Effective October 1, 2016) Refusal of inspection fee permit; appeals. (a) The Department of Revenue may refuse to issue an inspection fee permit under this article if the applicant or any principal of the applicant has done any of the following: (1) Had a motor fuel license or registration issued by this state or another state canceled for cause. (2) Been convicted of any offense involving fraud or misrepresentation. (3) Been convicted of any other offense that indicates that the applicant may not comply with this article if issued an inspection fee permit. (a)(1) Effective July 10, 2010, except as otherwise provided by this subsection, a person convicted of a crime that requires registration as a sex offender under Chapter 20A of Title 15, formerly Article 2 of Chapter 20 of Title 15 is prohibited from driving a commercial motor vehicle that requires a commercial driver license with a P or an S endorsement.
