Section 35-4-110 Instrument not recordable unless statement endorsed on it. No probate judge
shall receive for record or permit the recording of any instrument in which the title to real
property, or any interest therein, or lien thereon, is conveyed, granted, encumbered, assigned,
or otherwise disposed of, or any instrument relating to the organization, reorganization,
or dissolution of a private corporation, unless such instrument has endorsed on it a printed,
typewritten, or stamped statement showing the name and address of the individual who prepared
the instrument. If the instrument is in part composed of a printed form the person.preparing
the instrument shall be the person who filled in the blanks in the form or examined the entries

Section 45-49-120.14 Appointments. (a) Whenever a vacancy is to be filled by an appointment,
the appointing authority shall submit to the director a statement of the position and, if
requested by the director, the duties of the position and desired qualifications of the person
to be appointed, with a request that the director certify to the appointing authority the
names of the persons eligible for appointment to the position. The director shall then certify
to the appointing authority the names of the top 10 eligible persons on the appropriate register

transaction. In a consumer-goods transaction, the following rules apply: (1) A notification
of disposition must provide the following information: (A) the information specified in Section

Section 40-12-412 License - Application. Every person, firm, or corporation desiring to engage
in the business of an automotive dismantler and parts recycler shall apply in writing to the
Department of Revenue on a form prescribed by the department, which form shall contain: (1)

(3) A statement that the applicant's place of business meets federal, state, and local laws
concerning screening and beautification, which is a requirement to be licensed under this

Section 17-10-2 Provisional ballots; certification; tabulation; identification of ballots.
(a) A voter shall be required to cast a provisional ballot when: (1) The name of the individual
does not appear on the official list of eligible voters for the precinct or polling place
in which the individual seeks to vote, and the individual's registration cannot be verified
while at the polling place by the registrar or the judge of probate. (2) An inspector has
knowledge that the individual is not entitled to vote at that precinct and challenges the
individual. (3) The individual is required to comply with the voter identification provisions
of Section 17-10-1 but is unable to do so. If the voter's ballot becomes a provisional ballot
