Section 34-21-20.1 Submission and use of fingerprints for criminal history background check;
costs; confidentiality. (a) Each applicant for a multistate license as a registered nurse
or licensed practical nurse, and each applicant for initial licensure by endorsement as a
registered nurse or licensed practical nurse, shall submit a full set of fingerprints to the
board for the purpose of obtaining a state and national criminal history background check.
(b) Fingerprints obtained pursuant to subsection (a) may be exchanged by the board, the Alabama
State Law Enforcement Agency or any successor entity thereof, or any channeler approved by
the board, with the Federal Bureau of Investigation for the purpose of obtaining a state and
national criminal history background check. (c) The applicant or licensee shall be responsible
for all costs associated with the submission of his or her fingerprints and obtaining a state

Section 34-13-91 Application for examination; interview; criminal history background check.
(a) The applicant for an embalmer's license shall make application to the board. The application
shall be in writing and verified on a form provided by and addressed to the board and shall
be accompanied by the prescribed fee and by affidavits of at least two licensed embalmers
to the effect that the applicant is of good moral character and has met all qualifications
required for examination for license as prescribed by this chapter. The board may require

Section 34-24-160 Application; qualifications of applicant; fee. (a) Any person wishing the
right to practice chiropractic shall make application to the State Board of Chiropractic Examiners
in the form as the board may prescribe. (b) In addition to other requirements established

Section 38-13-4 Mandatory criminal history check notice; suitability determinations; checks
on subsequent activity. (a) Every employer, child care facility, adult care facility, the
Department of Human Resources, and child placing agency required to obtain a criminal history

of employment, or issuance of a license or approval or renewal thereof, and maintain in the

Section 34-13-51 Reciprocity of licenses. (a) The board may recognize and issue, without examination
