Section 34-21-123 Applications for licensure in a party state. (a) Upon application for a multistate
license, the licensing board in the issuing party state shall ascertain, through the coordinated
licensure information system, all of the following: (1) Whether the applicant has ever held,
or is the holder of, a license issued by any other state. (2) Whether there are any encumbrances
on any license or multistate licensure privilege held by the applicant. (3) Whether any adverse
action has been taken against any license or multistate licensure privilege held by the applicant.
(4) Whether the applicant is currently participating in an alternative program. (b) A nurse
may hold a multistate license, issued by the home state, in only one party state at a time.

Section 34-9-10 Application; licensure by credentials; special purpose license. (a) Every person
who desires to practice dentistry within the State of Alabama shall file an application prescribed
by the board. Notwithstanding the method of obtaining licensure or any particular requirement

Section 5-13B-82 Application to establish and maintain a branch or agency - Contents. (a) A
foreign bank, in order to procure a license under this article to establish and maintain an
Alabama state branch or Alabama state agency shall submit an application to the superintendent.

(a) No person may engage in the custom application of pesticides within this state at any
time without first procuring an annual license from the commissioner. The fee for such license
which shall accompany the application for a license shall be established by the Board of Agriculture
and Industries for the custom application of pesticides by the use of ground equipment. The

licensure through the compact shall file an application for an expedited license with the
