Section 8-26B-5 Registration as athlete agent; application; requirements; reciprocal
registration. (a) An applicant for registration as an athlete agent shall submit an application
for registration to the Secretary of State in a form prescribed by the Secretary of State.
The applicant must be an individual, and the application must be signed by the applicant under
penalty of perjury. The application must contain at least the following: (1) the name and
date and place of birth of the applicant and the following contact information for the applicant:
(A) the address of the applicant's principal place of business; (B) work and mobile telephone
numbers; and (C) any means of communicating electronically, including a facsimile number,

Section 8-7A-6 Application for license. (a) A person applying for a license under this
chapter shall do so in a form and in a medium prescribed by the commission. The application
shall contain all of the following information: (1) The legal name, the residential address
of the applicant if the applicant is an individual, the business addresses of the applicant,
and any fictitious or trade name used by the applicant in conducting its business. (2) A list

Section 27-31A-4 Risk retention groups not chartered in this state. Risk retention groups
chartered and licensed in states other than this state and seeking to do business as a risk
retention group in this state shall comply with the laws of this state as follows: (1) NOTICE

Lines Insurance Multi-State Compliance Compact Act is enacted into law and entered into with

Section 27-31B-3 Licensing. (a) Any captive insurance company, when permitted by its
articles of association, charter, or other organizational document, may apply to the commissioner
