Section 27-7-4.4 Fingerprints. (a) In order to make a determination of insurance producer license
eligibility, the commissioner is authorized to require fingerprints of initial resident applicants
for an insurance producer license and to submit the fingerprints and the fee required to perform
the criminal history record checks to the Alabama Department of Public Safety and the Federal
Bureau of Investigation (FBI) for state and national criminal history record checks. (b) The
commissioner shall require a criminal history record check on each initial resident applicant
for insurance producer license in accordance with this section. The commissioner shall require
each applicant to submit a full set of fingerprints, including a scanned file from a hard
copy fingerprint, in order for the commissioner to obtain and receive national criminal history

Section 34-24-338 Certificate of registration - Contents; change of address. Upon due application
therefor, by a licensee of the Medical Licensure Commission, and upon the payment of fees
required to be paid by this article, the commission shall issue to such applicant a certificate
of registration signed by the executive officer of the commission, which certificate shall
recite that such person is duly registered for the year specified. Such certificate of registration

Section 34-27-34 Who may serve as qualifying broker; responsibility of qualifying broker; change

Section 34-9-15 Annual registration; continuing education. (a) No person shall practice dentistry
or dental hygiene in the State of Alabama unless licensed or permitted by the board and registered
annually as required by this chapter. The secretary-treasurer of the board shall issue to

Section 40-17-168.8 Discontinuance of licensed business; payment of taxes. (a) A licensee who
discontinues in its entirety the business for which a license was authorized by this article
shall notify the department in writing at least 10 days prior to the time the discontinuance,
sale, or transfer takes effect and shall surrender the license to the department. The notice
shall state the effective date of the discontinuance and, if the licensee has transferred
the business or otherwise relinquished control to another person by sale or other means, the
date of the sale or transfer and the name and address of the person to whom the business is
transferred or relinquished. The notice shall also include any other information required
