Section 34-25-36 Refusal, suspension, reprimand, probation, or revocation - Surrender of license.
Upon the revocation or suspension of any license, the licensee shall forthwith surrender the
license or licenses to the board; failure of a licensee to do so shall be a violation of this
chapter and, upon conviction, shall be subject to the penalties set forth in this chapter.
At any time after the suspension or revocation of any license, the board shall restore it

Section 34-39-15 Violation as misdemeanor; penalty; forfeiture and revocation of license. Any
person who violates any provision of this chapter as set forth in Sections 34-39-4 and 34-39-13,

Section 5-18A-7 License - Issuance; display; payment of fees; surrender. (a) Upon the filing
of an application in the form prescribed by the supervisor, accompanied by the fee and documents
required pursuant to Section 5-18A-6, the supervisor shall investigate to ascertain whether
the qualifications prescribed by Section 5-18A-4 have been satisfied. If the supervisor finds

Section 34-24-217 Grounds for refusal, suspension, or revocation of license. (a) The board
shall refuse to issue a license to any person and, after notice and hearing in accordance
with its regulations and rules, shall suspend or revoke the license of any person who has:

Section 34-43-15 Complaints; grounds for suspension; revocation, etc., of license; penalties;
rulemaking authority. (a) Any person may file with the board a written and signed complaint
regarding an allegation of impropriety by a licensee, establishment, or person. Complaints
