Section 34-13-91 Application for examination; interview; criminal history background check.
(a) The applicant for an embalmer's license shall make application to the board. The application
shall be in writing and verified on a form provided by and addressed to the board and shall
be accompanied by the prescribed fee and by affidavits of at least two licensed embalmers
to the effect that the applicant is of good moral character and has met all qualifications
required for examination for license as prescribed by this chapter. The board may require
an applicant for an embalmer's license to submit to a personal interview. (b) Commencing on
October 1, 2017, in addition to the requirements of subsection (a), an applicant for an embalmer
license shall submit to the board a form, sworn to by the applicant, that contains the name,
date of birth, and Social Security number of the applicant, and two complete sets of fingerprints,

Section 25-12-11 Examination of inspectors. The examination for chief, deputy, special, or
owner or user inspectors shall be in writing and shall be held by the board or by an examining
board appointed in accordance with the requirements of the National Board of Boiler and Pressure
Vessel Inspectors, with at least two members present at all times during the examination.

satisfactory proof that he or she is a graduate of an accredited veterinary school and who

Section 45-39-40.19 Refusal, revocation, suspension of license or certificate; hearing. (a)
The board shall have the power to refuse, revoke, and suspend licenses and certificates, strictly

Section 28-3-52 Maintenance, examination and audit of books, records and accounts of board.
The books and records of the board shall at all times be subject to examination and audit
by the Department of Finance. The board shall keep a complete and accurate record of all its
actions and devise and install a system of accounts as the Department of Finance shall approve

Section 34-12-9 Licenses - Revocation; administrative fines; reissuance; replacement of lost
license. The board may revoke the license of any registrant who is found guilty by the board
of gross negligence, incompetency, or misconduct in the practice of forestry. The board may
