(a) An applicant for a license as a salesperson shall submit to the division, in the form prescribed, a written application for a license. The application shall set forth the following information: (1) The true name, date of birth, driver's license number, Social Security number, and home address of the applicant. (2) Each business or occupation engaged in by the applicant during the three years immediately preceding the date of the application, and the location thereof. (3) The previous experience of the applicant as a commercial telephone seller or salesperson.
