Applicants for license under this part shall file with the license commission a sworn application in writing, in duplicate, on a form to be furnished by the license commission which shall give all of the following information: (1) Name and physical description of the applicant. (2) Date of birth and drivers license number of the applicant. (3) Full local address of the applicant, and the length of time the applicant has resided within the county. (4) If employed, the name and address of the employer, together with credentials establishing the employer-employee relationship.
