Section 10A-1-3.07 Certificate of existence or registration. Unless provided otherwise
in a chapter of this title governing an entity: (a) The Secretary of State, upon request and
payment of the requisite fee, shall furnish to any person a certificate of existence for a
filing entity if the filing instruments filed with the Secretary of State show that the filing
entity has been formed under the laws of this state. A certificate of existence shall reflect
only the information on file with the Secretary of State. A certificate of existence must
state: (1) the filing entity's name; (2) that the filing entity was formed under the laws
of this state and the date of formation; (3) whether the filing entity has delivered to the
Secretary of State for filing a certificate of dissolution; (4) whether the filing entity

Section 22-19-173 Rights and duties of procurement organization and others. (a) When
a hospital refers an individual at or near death to a procurement organization, the organization
shall make a reasonable search of the records of the Alabama State Law Enforcement Agency
and any donor registry that it knows exists for the geographical area in which the individual
resides to ascertain whether the individual has made an anatomical gift. (b) A procurement
organization must be allowed reasonable access to information in the records of the Alabama
State Law Enforcement Agency to ascertain whether an individual at or near death is a donor.
(c) When a hospital refers an individual at or near death to a procurement organization, the
organization may conduct any reasonable examination necessary to ensure the medical suitability
of a part that is or could be the subject of an anatomical gift for transplantation, therapy,

Section 33-5A-3 Duty. (a) Except as provided in Section 33-5A-5, a person who
removes a vessel pursuant to this chapter shall do all of the following: (1) Give written
