Section 26-10A-11 Consent or relinquishment. (a) A consent or relinquishment shall be in writing,
signed by the person consenting or relinquishing, and shall state the following: (1) The date,
place, and time of execution. (2) The date of birth or if prior to birth expected date of
birth of the adoptee and any names by which the adoptee has been known. (3) The relationship
of the person consenting or relinquishing to the adoptee. (4) The name of each petitioner,

Section 26-2A-156 Claims against protected person; enforcement. (a) A conservator may pay or
secure from the estate claims against the estate or against the protected person arising before
or after the conservatorship upon their presentation and allowance in accordance with the
priorities stated in subsection (d). A claim may be presented by either of the following methods:
(1) The claimant may deliver or mail to the conservator a written statement of the claim indicating

Section 27-2-32 Hearings - Appeals. (a) An appeal from the commissioner shall be taken only

Section 45-8A-71.13 Employees, officers, or officials removed, discharged, or demoted; proceedings;
appeals. (a) The governing body of the city may remove, discharge, or demote any employee,
officer, or official of the city who is subject to this part and who is directly under the
