Section 45-49-40.17 Barber school prerequisites. (a) An application for a license and approval as a registered school or college of barbering shall contain, under oath, all of the following: (1) The full name of the applicant. (2) The residence of the applicant, and if an association or corporation, the same information of the members of the association and of the stockholders and directors of the corporation. (3) The exact location where the school or college is located or proposed to be located. (4) Whether or not the school or college is owned or leased, and if leased, the name and residence of the owner, or if an association or corporation, the same information of the members of the association and of the directors and stockholders thereof. Section 34-14C-4 Licensure; inspections. (a) Except as otherwise provided in this chapter, a home medical equipment services provider shall be licensed annually by the board before the provider may engage in the provision of home medical equipment services. In Alabama, when a single business entity provides home medical equipment services from more than one location within the state, each such location shall be licensed. A provider of home medical equipment services that has a principal place of business outside this state shall maintain at least one physical location within this state, each of which shall be licensed. Section 34-30-26 Term; expiration date; renewal; inactive status. (a) All licenses and certificates under this chapter shall be effective when issued by the State Board of Social Work Examiners. (b) All licenses and certificates issued by the board shall expire on the last day of the month in the calendar year that is exactly two years from the calendar year and month in which the license or certificate is issued.
