Section 27-45A-4 Licensure of pharmacy benefits managers. (a) (1) Effective January 1, 2020, to conduct business in this state, a pharmacy benefits manager must be licensed by the commissioner. To initially obtain a license or renew a license, a pharmacy benefits manager shall submit all of the following: a. A nonrefundable fee not to exceed $500. b. A copy of the licensee's corporate charter, articles of incorporation, or other charter document. c. A completed licensure form adopted by the commissioner containing: 1. The name and address of the licensee. 2. The name, address, and official position of an employee who will serve as the primary contact for the Department of Insurance. 3. Any additional contact information deemed appropriate by the commissioner or reasonably necessary to verify the information contained in the application. (a) Each license or permit issued by the Commissioner of Conservation and Natural Resources or his or her authorized agent shall state the name of the applicant. Section 27-1-16 Standard health insurance claim form; electronic claims form; various claim forms. (a)(1) The Commissioner of the Department of Insurance shall prescribe a standard health insurance claim form to be used by all hospitals. The forms shall be prescribed in a format which allows for the use of generally accepted diagnosis and treatment coding systems by providers of health care and payors. Section 34-21A-18 Annual renewal of license. (a) A license is only valid during the calendar year indicated on the license. At the end of that calendar year, the license will expire unless it is renewed by the licensee. All licensees shall make application for license renewal at least 30 days prior to the expiration of a current license. (b) Specific license renewal application forms and license renewal requirements shall be developed by the board. License renewal requirements shall include, but not be limited to, continuing education and training requirements for each level and type of license and evidence of adequate bond.
