Section 27-19-9 Mandatory policy provisions - Claim forms. There shall be a provision as follows:
"Claim Forms: The insurer, upon receipt of a notice of claim, will furnish to the claimant
such forms as are usually furnished by it for filing proofs of loss. If such forms are not
furnished within 15 days after the giving of such notice, the claimant shall be deemed to
have complied with the requirements of this policy as to proof of loss upon submitting, within
the time fixed in the policy for filing proofs of loss, written proof covering the occurrence,

Section 27-20-5 Blanket disability insurance - Power to issue; filing requirement; mandatory
policy provisions. Any insurer authorized to write disability insurance in this state shall
have the power to issue blanket disability insurance. No such blanket policy may be issued
or delivered in this state unless a copy of the form thereof shall have been filed in accordance
with Section 27-14-8. Every such blanket policy shall contain provisions which, in the opinion
of the commissioner, are at least as favorable to the policyholder and the individual insured
as the following: (1) A provision that the policy, including endorsements and a copy of the
application, if any, of the policyholder and the persons insured shall constitute the entire
contract between the parties, and that any statement made by the policyholder or by a person

Section 25-5-8 Employers' options to secure payment of compensation. (a) Option to insure risks.
An employer subject to this chapter may secure the payment of compensation under this chapter
by insuring and keeping insured his or her liability in some insurance corporation, association,
organization, insurance association, corporation, or association formed of employers and workers
or formed by a group of employers to insure the risks under this chapter, operating by mutual
assessment or other plans or otherwise. Notwithstanding the foregoing, the insurance association,

Section 27-14-27 Acts not deemed waiver of provisions or defenses. Without limitation of any
right or defense of an insurer otherwise, none of the following acts by, or on behalf of,
an insurer shall be deemed to constitute a waiver of any provision of a policy or of any defense
of the insurer thereunder: (1) Acknowledgment of the receipt of notice of loss or claim under
the policy; (2) Furnishing forms for reporting a loss or claim, for giving information relative

Section 27-19-8 Mandatory policy provisions - Notice of claim; notice of disability continuance.
There shall be a provision as follows: "Notice of Claim: Written notice of claim must
be given to the insurer within 20 days after the occurrence or commencement of any loss covered
by the policy, or as soon thereafter as is reasonably possible. Notice given by, or on behalf

Section 27-1-17 Limitation periods for payment of claims; overdue claims; retroactive denials,
adjustments, etc.; penalties. (a) Each insurer, health service corporation, and health benefit
plan that issues or renews any policy of accident or health insurance providing benefits for
medical or hospital expenses for its insured persons shall pay for services rendered by Alabama
