Section 8-19A-5 Licensing; application for license. (a) Prior to doing business in this
state, a commercial telephone seller shall obtain a license from the division. Doing business
in this state includes both telephone solicitation from a location in Alabama and solicitation
from other states or nations of purchasers located in Alabama. (b) An applicant for a license
as a commercial telephone seller shall submit to the division, in the form prescribed, a written
application for the license. The application shall set forth the following information: (1)
The true name, date of birth, driver's license number, Social Security number, and home address
of the applicant, including each name under which he or she intends to do business. (2) Each
business or occupation engaged in by the applicant during the three years immediately preceding

Section 34-13-120.1 License requirements; certification. No person may conduct, maintain,
manage, or operate a cremation facility unless licensed to do so by the board. The board may
issue a license to practice as a cremationist after the applicant has satisfied all of the

Section 8-26B-5 Registration as athlete agent; application; requirements; reciprocal
registration. (a) An applicant for registration as an athlete agent shall submit an application
for registration to the Secretary of State in a form prescribed by the Secretary of State.

Section 37-3-25 Penalties for violations. (a) Any person knowingly and willfully violating

Section 45-9-82.27 Costs and fees. (a) An applicant may be assessed a fee when the applicant
is approved for the program. The amount of the assessment for participation in the program
shall be in addition to any court costs and assessments for victims or drug, alcohol, or anger
management treatment required by law, and are in addition to costs of supervision, treatment,

(a) The following persons or entities shall be permitted access to the information in the
controlled substances database, subject to the limitations indicated below: (1) Authorized
