Section 34-13-91 Application for examination; interview; criminal history background check. (a) The applicant for an embalmer's license shall make application to the board. The application shall be in writing and verified on a form provided by and addressed to the board and shall be accompanied by the prescribed fee and by affidavits of at least two licensed embalmers to the effect that the applicant is of good moral character and has met all qualifications required for examination for license as prescribed by this chapter. The board may require an applicant for an embalmer's license to submit to a personal interview. (b) Commencing on October 1, 2017, in addition to the requirements of subsection (a), an applicant for an embalmer license shall submit to the board a form, sworn to by the applicant, that contains the name, date of birth, and Social Security number of the applicant, and two complete sets of fingerprints, for completion of a criminal history background check. (a) No license shall be issued by the board without examination of the applicant for the purpose of ascertaining his or her qualifications for such work, except those licenses issued pursuant to Section 34-21A-17. Section 34-25A-12 Fees. (a) The board shall promulgate rules and regulations for all licensure fees, registration fees, renewal fees, and accreditation fees. Section 22-18-4 Fees of licenses; disposition of funds; qualifications for EMSP licensure. (a) In addition to all other fees now payable, the Board of Health shall establish, by rule, a fee for EMSP and air or ground provider service licenses. Each license issued to an EMSP shall be valid for a period of 24 calendar months. Each license issued to a provider service shall be valid for a period not to exceed 12 calendar months. (a) The board may issue a temporary license to practice veterinary medicine to an unlicensed applicant providing the applicant meets all conditions and requirements of this article relating to qualifications of applicants for license to practice veterinary medicine.
