Section 13A-6-21 Assault in the second degree. (a) A person commits the crime of assault in
the second degree if the person does any of the following: (1) With intent to cause serious
physical injury to another person, he or she causes serious physical injury to any person.
(2) With intent to cause physical injury to another person, he or she causes physical injury
to any person by means of a deadly weapon or a dangerous instrument. (3) He or she recklessly
causes serious physical injury to another person by means of a deadly weapon or a dangerous
instrument. (4) With intent to prevent a peace officer, as defined in Section 36-21-60, a
detention or correctional officer at any municipal or county jail or state penitentiary, emergency

Section 27-1-16 Standard health insurance claim form; electronic claims form; various claim
forms. (a)(1) The Commissioner of the Department of Insurance shall prescribe a standard health
insurance claim form to be used by all hospitals. The forms shall be prescribed in a format
which allows for the use of generally accepted diagnosis and treatment coding systems by providers
of health care and payors. The standard form shall be accepted and used by all insurers doing
business in the State of Alabama and by all state agencies which pay providers of health care

competent adult may execute a living will directing the providing, withholding, or withdrawal

Legislature finds and declares the following: (1) The citizens of this state rely upon health
insurance to cover the cost of obtaining health care and it is essential that the citizens'
